The Office of the Commissioner for Persons with Disabilities is starting a survey of all persons with disabilities, in the NCT of Delhi with a view to preparing a detailed data base of the primary information of persons with disabilities. This data base will be used by this office and other Govt. departments to help persons with disabilities through better formulation of schemes and in reaching the existing assistance being made available to them by Government. The form below has to be filled up and a unique number generated. This number should be retained for accessing your form and updating it as needed. Each registered users should update his/ her status, every twelve months in case of any change.
